ツアー開始時に以下の内容の参加同意書にご記入いただきます。
☐ 妊娠中のお客様、また飲酒されている方はツアーに参加できません。

・Pregnant ・Drinking alcohols
☐ 悪天候・災害、参加者の体調等により、ツアーのコース変更・中止をする場合があることを理解しています。
I understand that the tour course may be changed or canceled due to bad weather, disasters, physical condition of participants, etc.
☐ ツアー中はガイドの指示に従い、安全が最優先であることに同意します。
I follow the guide’s instructions during the tour and I agree that safety is paramount.
☐ 私は、主催団体の選択する医療機関への移送・治療・緊急の処置に同意し、私に要した治療費を負担致します。
I agree to transfer, treatment and emergency treatment to the medical institution of your choice and will bear the cost of treatment for me.
☐ 私は、万が一、身体的傷害、損傷が生じた場合の補償、主催団体及び他の参加者に対して責任を問わず、自己の責任において処理致します。
I understand that compensation in case of injury in the unlikely is covered by organizer’s insurance coverage.
I will handle it at my own risk, calless of the responsibility of the organizer and other participants.
☐ 私は、私の参加するツアーが自然を利用して行なわれるものであり、自然による危険性、ツアー中の予測困難な事由が起こり得る場合もあること、
また、参加中に自分又は他人の行為により身体上の損害、危険を受ける場合もあることを十分に理解･認識した上で同意し、参加致します。
